CAI is an approved provider for most
major insurance carriers and health plans. We will submit your claim--and
any secondary claims--to any insurance company under the benefit coverage
you specify, including workers' compensation, auto insurance, and personal
injury. This is one of the levels of service that set CAI apart.
Before your exam, a CAI representative may call to assist you with
pre-registration. Please arrive 20 minutes before your scheduled
appointment time to verify your registration information and complete
other necessary exam forms.
To streamline the registration process and minimize your
personal expenses, it is important to take the following steps
before your appointment:
Step 1: Find out if your policy or plan requires
a preauthorization or referral. If so, contact your insurance company
and find out what you need to do to obtain one.
Step 2: Give CAI complete insurance information,
including insurance card, name and address of your plan or insurer(s),
name and address of your claims payer (if different from insurer),
your ID and group numbers, and the name of the policyholder or
subscriber. If applicable, for workers' compensation, automobile,
or personal injury cases, we require the name and address of your
attorney as well.
Ultimately, the patient is responsible for payment of the account, including
any balance not covered by insurance. We will send you a balance statement
for any amount you owe after insurance. Please contact your insurance
company if they have not paid for your procedure after 30 days.

Q: What do I need
to bring with me to facilitate the payment process?
A: Bring your complete insurance information, including your insurance
card, ID number, and group number. In addition, for workers' compensations
claims and automobile injuries, bring your claim number, the date
of injury and the name and address of your attorney.
Q: Do I need preauthorization from my heath care
policy or plan?
A: Some plans do require preauthorization. Check with yours to
see if this is necessary. Your insurance company can also answer
any questions you may have about what is covered and what is not.
Q: Does CAI file secondary claims as well as primary
ones?
A: Yes, we do.
Q: I don't have medical insurance...Can I get a
price quotation on my procedure?
A: If you do not have medical insurance and/or you are concerned
about paying for your procedure, call the CAI business office.
We'll give you an estimate and work with you in setting up a payment
